Information on Insurance

We are a contracted provider with many insurance carriers.
We encourage you contact your insurance carrier to learn what your coverage is.

You may or may not be required to make a co-payment at the time of service depending on you insurance carrier & coverage.

You may also be responsible for a percentage of the bill after insurance pays.

Our rates are based on the Medicare allowed rate for our geographic area.

In the event that your benefits are exhausted for the calendar year, or your services are not covered, we accept all major alternative forms of payment.

Body Balance and Weight loss services are not typically covered by insurance.

If you have been involved in a motor vehicle accident, we will provide services under PIP coverage.

We will ask you to sign a lien in order to assure direct payment in full upon the settlement of your claim.

If the PIP coverage becomes exhausted, we will bill your medical insurance untill your benefits have been exhausted.

Thereafter, we can continue to provide you treatment with a $50 co-pay.

All unpaid balances are subject to interest accrual.

If you have anyquestions about billing or payment options, our billing staff will be glad to discuss those options with you.

A prescription, referral or prior authorization may be required by your insurance.
This should be requested by your primary care provider.
