IMPORTANT REFERRAL INFORMATION
Group Health Cooperative (GHC) has very strict rules regarding the referral process
Please reveiw the Group Health referral rules prior to scheduling.
It will be very important for you to verify your referral/prior authorization has been approved prior to arrival.
A referral/prior authorization can only be created by your Primary Care Provider (PCP)
The PCP must follow the policies and guidlines setforth by GHC.
The referral must go through the GHC utilization Reveiw process prior to your first appointment.
All GHC clients will be required to complete the Non-Covered Service Agreement Form upon arrival for appointments.
